he didnt do any test or xrays

it could also be bursitis this is an inflammation of the bursa, cortisone injections and heat/cold compresses are used for treatment. I have this and this is the treatment I get. you are right to question the doctor he cannot see a tear without an x-ray or CT-scan, he is probably just using an educated guess, ask your wife if the pain feels like it is really tight pulling sensation when she gets the pain if she says yes then it is bursitis.
