He wants to know should she be in a hospital? I know normal is 120/80, and 140 is moderately high. 200 is extremely high, but to me does not mean on deaths doorstep, but she is on BP meds already. I think she should be checked out for why her BP is so high even on meds. Please, professionals only.

actually, the normal BP is down to 100/60 for most individuals...some docs still use the 100 + your age for the systolic and 1/2 of that number for the diastolic...was the BP accurate? did they take it more than once? is the cuff calibrated properly? did they take her BP right after 10 cups of coffee, or a marathon? yes, i know she's 82...but you get the point...why was her BP up...did she forget her meds for a few days and not tell anyone...
if she is on meds and her BP is still that high...she needs to see her doc today...if you call the office and tell them what is going on, they may say, just go to the ER...if you show up, then they are obligated to see you and initially treat you...you may still go to the ER, but that would depend on the conversation you have with the doctor
if you call an ambulance, they will NOT take her to the docs office, they will automatically go tothe ER.

I am assuming that 200 is the systolic BP. Yes, it is high and she should definitely be under the superision of a physician. She may have had this BP for a while. If she were my patient, I would take this seriously and hospitalize her and try to reduce her blood pressure slowly back to normal or near normal range. Her body may be so use to this BP that lowering too fast may be deterimental. Keeping it elevated will surely cause an MI or stroke. You can never tell, she could last years on a SBP of 200 or she could have a cardiovascular event tomorrow. Is she watching her salt intake, does she take her meds regularly, has her MD tried different combinations of anti-hypertensives to better control her BP? I would recommend that she see her doctor ASAP.

Patients with renal damage are difficult to get BP down. Over 200 is inviting a stroke and further effort should be made to reduce it.

Yes, she needs to be checked. Her pressure is dangerously high. With a pressure that high, she could have a stroke. What is her diastolic, sometimes that is an even better indicator. Have her looked at !!
