Caicos Turkey's was almost right on, but 'in aeternum' is an adverbial phrase in its own right. In takes the accusative case in this instance, so you cannot change aeternum to aeterne (a typical adverb form). That poem by Catullus is one of my favorites. Very sad, but beautiful.
The singular/plural forms I have listed above are referring to the number of departed people, not the ones left behind. Don't ever use an online translator for Latin - they are really awful. What they put back out as "Latin" is just a mess.

Praemature ademptus mihi, amatus in aeterne.
This is the masculine singular. It would be "Praemature adempta" and "amata" for the feminine singular, "Praemature adempti" and "amati" for more than one person, unless it were several people, all of them female, in which case :"Praemature ademptae" and "amatae".
Likewise, "mihi" is singular. If several people are mourning the, proper form is "nobis".
Literally, it is "too soon snatched from me/us, loved eternally."

quandoquidem fortuna mihi tete abstulit ipsum,
heu, miser indigne frater adempte mihi!
