hahaha. don't let her be used to that thing... why?
1) you guys are just good in saying that for the first few months or years of relationship. after couple of months/years, you fade. that's how your focus change. right girls?
2) if you let her be used in hearing you saying that, and she likes it. she would definitely long for it all the time. and when the time comes you can't say it, you quarrel. hehehe.
3) if she doesn't like it she will be annoyed. she will think you are overusing the word and she cant easily figure out when you say it with sincerity or not.
so my advice to you, leave some mystery to her. fact: marriage couple that lasts 20-50 yrs are those that kept mystery in the relationship (not the mystery of other girls... that's a diff. story!)

She might get a bit annoyed or suspicious if you tell her EVERY time, depending on your girlfriend. You should actually know her better than we do.

On the other hand, you shouldn't really change it now as if you will, she will notice the change and get even more suspicious. Just don't worry about it. Or ask her yourself, it won't hurt your relationships.

It depends. Do you say it from the hearth or is it automatic like a "thank you" and "your welcome". If it's automatic, it won't be long before she's able to tell. I'm pretty sure she won't like that. Of course, if you say from the hearth, well, I don't think any woman will get tired of too much hearth.
