It could mean he was the first one she had sex with. But it could mean that it was the first one she dated exclusively for an extended period of time, met his family, went to his house, knew that on Saturday night she would be with him, etc.

Sometimes but not usually. When a girl says that a guy is her first real boyfriend she means that she has had guys interested in her before and maybe even dated a couple of them but this is the first one she's serious about. So in other words he likes her and she likes him too. They are not seeing other people which allows her to really connect with this guy.

I dont know about all those other girls but for me no. When I say " he was my first real boyfriend " what I mean is that he was my first longterm relationship where I invisioned a future and felt " in love " weather it was real or not. Alot of girls loose thier virginitys to scum bags they later regret these days unfortinutly and alot of girls have had other boyfriends before the " first real boyfriend " but the first real boyfriend is usually the first relationship that lasts longer then 3 months, grows a deep bond and meets your friends/family.

A "first real boyfriend" is when both guy and girl can say "I love you" or w/e it is that shows u both care a lot for each other.
Im in a relationship right now with my first boyfriend. He is the first guy that I like to hold hands with and He is teh one I yearn for when I feel sucky
