I am looking for the forms that I would give to the parents asking what they are allergic to, if they need any med. emergency contacts ect.. does anyone know where i can find a printable form or even an example of this?

Also, it is mandatory that you have a medical consent form for EACH child that has been notarized in case you have to take a child to the hospital--they won't treat the child otherwise.

Most computers have a program where you can create this type of form along with many others. Microsoft Works usually has task forms that you can create. Also there is a program called Photo Explosion that has some good one's as well. If you do not have any of these programs then may I suggest going to a place that does such as Office Max, Office Depot.. I'm not sure if you have one of those in your area but there may be one under a different name that does the same thing!

This website seems to offer a lot of pre-made forms. You may need to right click on the entire form to highlight it and copy it- then in a microsoft word or something like it right click and paste. From there you can change the words to suit your situation and VIOLA! Its a neat website.
