This is going to be gross, but someone's gotta tell ya. It might be your tonsils.

Are your tonsils producing too much protein? Do you have sinus problems? Sometimes you can get a thick build up in the little crevaces around your tonsils and that makes for a very foul odor. You can even press them with a Q-tip and smelly-chunks pop out of them.

Anyway, this happened to my girlfriend who had mononucleosis in highschool and we found out that people can have this problem with or without mono.

First, be aware that you could be suppressing the symptoms of an infection of the gums by doing good hygiene. A dentist told me I had periodontal disease, because I didn't take care of my teeth. I said I bushed, I flossed, and had no symptoms. He said that that was because I took care of my teeth!

Second, some bad breath is caused by bacteria on the tongue, especially way back, so you should try scraping your tongue, like with a spoon, and brushing it...see if that helps. But for sure, you should consult a dentist.

Sometimes bad breath or halitosis doesn't just come from your mouth and teeth. Especially if you say that you have good oral hygiene. It could signify an internal problem (esophageal problem, lung problem, etc) that you need to ask your doctor about. Some people who have bronchitis, esophageal pouches, etc may manifest bad breath as a symptom. Good luck!

cut back on eating meat, it takes 72 hours to digest meats, and it starts to rot in your gut, thats where bad breath comes from, not your teeth, eat more greens, the natural breath control,if man were meat eaters our intestines would be only 27 inches long, instead of 27 feet. moderation in everything, more fruits and veggies, vegetarians have nice smelling bodies and breath, sexy in fact. i like a steak once and a while too

A few seeds of cardamon chewed for a brief while will clear your mouth of bad breath. Chew parsley leaves, which are sure to keep your breath fresh. More useful remedies at http://useinfo5.blogspot.com/

Talk to your doctor about it. It could be acid reflux, or other issue with your stomach. I remember hearing that cloves somehow help too but ask your doc before you eat a bunch of them. Do you brush your tongue too?

Sometimes it is a sign of the body trying to eliminate "stuff" that's not getting out thru the normal pathways. Are you eliminating well enough otherwise ? How's your diet? Food allergies ?
