You can extend your visa due to special circumstances. In that case you or somebody who needs you to stay here would have to file a petition to immigration authorities. You would have to file it before your visa expires.
Otherwise you would have to return to your country. Then you might be able to come and visit again.

I don't know if they extend tourist visas without unusual circumstances, but definitely I would say before it expires, since at that point you would be deportable.

Do it before it expires. if you don't you will be here illegally.
