The best way to get the INS forms is contact your local Congressman. Go to the House of Representative website and find out who your Rep is through the Zip Code search.

Once you have the name and number, contact them and explain your situation. They should be more than happy to help direct you, because that is what they get paid to do!

Trust me on this, I used to intern at a Congressman's office, and this is the best way to go to get an answer.

You can get a packet with eveything you need from your local CIS office.
