If you have engaged an agency to find you work,
it is the agency's responsibility to get the work permit for you from your future employer.
If however, you sought your employer privately, you will
have to request them to file the application on your behalf.
You do not get the permit yourself.

The company is in charge of getting the work permit for you. If they aren't doing their job, then you should go after them for it.

Are you a minor? If so you get it from the school. From what I remember, and this goes back years, you get the forms and your parents fill them out then you return it.

You get it. I got mine from my counselor when I was in middle school. You will probably be able to get it from your counselor as well.
