I would just write something from the heart as that is much a better than a poem.

It sounds like your Golden lived a long life as they dont usually live much past 12 years. I have a 10 year old and know that we wont have her for too much longer.

Sorry for your loss

I am so sorry. It must be really hard. Just write a normal thank you note. It doesn't need to be a poem or a formal letter, just a thank you note. Like...
Dear Dr. Dude and Nurse Chick,
These last few weeks have been very hard on me and my family. We lost a very important member of our family yesterday. She lived a happy, joyful, and healthy life thanks to you. With out your help, my beloved (put dogs name here) would not have lived this long. She died surrounded by people who loved her and cared for her. I just want thank you for everything you did for my dog and me.

Something like this. Put your heart in it! It will make you feel better too.

I know how you feel, I lost 1 week before Christmas my dog last year, I cried like a baby, I'm still in mourning for him. If you want to I can help you write a poem, I'm a poet or a Thank You for you. And more, write me.

I just wanted to say I am sorry. I feel like crying because I know your pain. Sorry that I do not have a website for you.I would use words that came from my heart even if it isnt the best poem or verse. Vets dont get letters to say thanks often. It would mean alot if you just simply wrote thank you
