-He's rude or impatient with the wait staff.
-He insults (or says something derogatory about) anyone unnecessarily.
-Mentions his jail time.
-Asks if I want him to pay. (He should either offer to pay, or offer to split the bill, but asking what *I* want him to do is rude, and doesn't leave me any polite option except to pay my share.)
-Asks if I think I'm fertile.
-Is too aggressive about the kiss or fondling, and isn't paying enough attention to my body language to realize I'm not equally enthralled with this kiss.
-Doesn't offer any conversation, gives only the barest answers to any personal questions, and then resorts to imitating me (I sniff, he sniffs; I cross my legs, he crosses his legs; I start to speak, he repeats my words).
And yes, I've had all of these happen to me within the past year or two, from adult men who ought to know better.

Takes calls at the dinner table or in a movie theater. Burps and doesn't say excuse me. May bump into someone they know, and does not introduce me, even as a friend. Forgot to put on or ran out of antiperspirant. Has food caught in their teeth or braces. Has snot still in their nose. Missed their lips when applying lipstick. Too much perfume is a killer. A run in her stockings. Talks of her ex (good or bad). Bumps into someone she knows and forgets that shes on a date with me. Is loud and obnoxious. Leaves articles over my house (lipstick case, or anything else that would give notice that a woman was present in my house). Bad breath. Ordering the most expensive thing on the menu when normally she wouldn't order if she was alone. Lipstick on the teeth. Dirt in fingernails. Feet that look Homer Simpson's, The Grinch that stole Christmas, Fred Flintstone's or like a herd of wild buffalo trampled over them. Ashy elbows or feet. Talking highly over yourself.

talk on the phone to long.. checking her looks out, worried what i think if she eats this or that.. so orders a salad, doesnt talk much, talks about her ex, look like she walked off the corner, meets her friends ask them to stay with us on our date, doesnt want to make any decisions regarding the date..ex. movie, food..ect.., women should never do any of these things.. these are the top ones.. and i hate the fact that some women think dates have to cost so much.. whatever happend with a picnic at the lake.. or park.. its like they forget that, that can be fun too..

* Doesn't ask me questions, or manages to steer whatever question he does ask me back to himself ("What's you're favourite food?" "I really like a good pasta in olive oil." "Well, I like hamburgers. There was this one hamburger . . . blah blah blah"
* Pulls the "I'm totally broke" routine AFTER ordering. If you don't have money, I'll cover, but at least give me a warning before you order 50 dollars worth of food!
* Appears obbsessive in talking about how he's "totally over" his ex. No seriously, totally over her. Doesn't care about her anymore. Doesn't even think about how they used to, etc. Did he mention that he's totally over her, and the way she cheated on him?
* Walks ahead of me. Just don't like it.
* Anything having to do with a cell phone unless its putting my number into it.
And the number one thing I HATE:
*Over use of the phrase "Normally, I never do this but . . ."
Unless we are eating live monkies on a snow mobile in the Arctic, I don't buy the "I never usually do this." Because usually, the "normally never do this" line is used to make a person seem like less of a sleeze.
