If she is outgoing and would like attention, then maybe a bunch of mylar birthday balloons for her porch railing, bedpost, wheelchair, whatever as a conversation starter.
A nice card with something heartfelt written large enough for her to read it. Sit with her and let her reminisce. Ask her to tell you about the good times she remembers. At age 90, someone usually does not need more 'stuff', altho I can picture getting her a nice top to wear. I think the perfect idea will come to you..

My Nana is not that old yet, but a few years ago I got a HUGE old photo / picture frame about the size of a poster from a second hand shop, went through her photo albums (when she wasn't looking) and made copies of the most important, significant photos of her life and other family members. I made it into a collage – she loved it, it was personal and sentimental., She is in a nursing home now and has it hanging up in her room opposite her bed so she can see it everyday.

If she enjoys watching movies, you could buy her a dvd...or simply a nice bouquet of flowers that always brings a smile to everyone. Or if she likes music, buy her a cd of the type of music she listens to. With a nice card, whichever present you choose.

I had a photo of her children ( when they were young) blown up and set in an antique looking frame. She just loved it. It was the only present that made her both cry and laugh.

well i work in a nursing home and i've learned that the elderly just stash their gifts and give them away to other people. the one thing i see them cherish alot is photos, something new. get some photos taken of you and your family or someone close to them and give that to them. anything else really just goes to waste. ( you don't know how many gifts these people try to give me at work)
