If it hasn't been changed by now.Do so soon as it should have been changed at 60,000 to 90,000 miles.You do not want it to break all the way as it will cause more damage to the valves and pistons by causing them to be 'out of time' .

Don't wait any longer. If you have no records of it ever being done, get it done very soon. 60,000 or 90,000 miles is recommended for cars with a timing belt.
I had an Acura Legend that I bought used and with no maintenance records. I was driving along and the belt let go. Engine stops and you have to get it towed somewhere. I don't know if the Toyota V6 is an "interference" engine or not, but the Acura was and it bent some of the engine valves.
Mine was under warranty, but if your Sienna isn't, it could be very costly if the belt breaks and bends the valves. Even if it isn't an interference engine, you still don't want the engine stopping suddenly on you.
Usually around $300-700 for a timing belt change. Call your local Toyota dealers (the price varies, so shop around), a local mechanic or even a Toyota only repair shop, if one exists. Ask if it involves changing the water pump too. Because if the water pump seizes, it will snap the belt too.

You are living on borrowed time.
The Timing belt should have been changed by now, but then again it is a Totota, so it will last.
Prevention: Change before it brakes, it is much cheaper, if you don't the repair will cost alot more than just 200 bucks. It will cost 900 to 1200 depends what else brakes along with it. Something always breaks.

If its never been done then your on borrowed time,60-90,000
and while your in there would recommend a water pump also
it runs of the timing belt, when you have the timing belt of its really easy to get to.

Do it now or you will have a lot to regret if it snaps on you while running. Should have been changed around 50k ago.
