Web MD says if it's under an inch long, leave it, over an inch, poke a hole in the side with a sterile needle, drain it, but do not remove the blister skin. Clean the area, try to leave it uncovered if you can in order to heal faster.

i would heat a needle to red hot, after it cools a little, put it in alcohol, and wipe your toe with alcohol. pierce it just enough
to get the puss out. put an antibacteral lotion or cream on it and put a bandaid or gauze around it and definitely see if you can pad it underneath on the blister so the chance of a callus
is lowered. if you don't take care of it, you will see it open after it has and can cause an infection, that you definitely
don't want.

Lance it and gently pull away the loose skin. Use peroxide or alchohol and clean it. (OUCH) It'll sting for a minute, but you need to get the germs out. Bandange it and make sure you wear sensible shoes for the next couple of days.
Change the bandage and clean it again with peroxide or alchohol at least once a day until it heals.

Leave it alone. If you pop it, you increase chances of infection. If it breaks on it's own, clean the area well with soap and water, put an antibiotic ointment like Neosporin on it and cover it with a Band-Aid.

I always stick them with a pin and drain it and cover it with a bandaid so nothing gets in it to infect it. The next day often the extra skin needs to be cut away once the skin underneath it is not so tender.
Peace!

If it is in a spot where its going to pop anyway. I'd pop it then put some neosporen (anti infection cream. Spelling?) on it, and a good band aid. If you keep it clean there is probably no chance of infection.
