I would really like to make another project (e.g hospitals) but since $1 million ain't that enough & not that much now a days so I will just give the money to the charitable institution for them to buy foods & some other needs...

I'd see if I could fund a series of community-wide clean-ups, calling on volunteers to provide the labor and trying to use the money mainly for supplies. (I'm now assuming that my office has enough staff or volunteers to coordinate this project - and that might not be the case. But let's just pretend.)
I know that planting trees, picking up garbage, painting over graffiti and boarding up abandoned houses isn't exactly a "big splash" project. But I think that cleaning up dozens and dozens and dozens of eyesores - and engaging the people who live right there to be part of the solution - can be really powerful. It isn't politically divisive, and the nature of the work means that lots of people would benefit - business owners, schools, churches, individual residents. And I think the answer could work in nearly any setting - maybe a really rural community doesn't need to clean up graffiti'd bus shelters the same way that my neighbor does, but we've all got room for improvement.
Any chance you have that million to spend? Today's a great day to paint over some spray-painted garbage cans on my corner! ;)

A Community Garden
Having a large vegetable garden for people to use and benefit from would be a good investment. People could have a meaningful activity and they could benefit from the fruits of this activity. Perhaps such an endeavor could be an alternative to programs like WIC or food stamps for some people(by choice). Gardening can be a family activity. A green house could be build for year round harvesting.

probably have the city council come up with
several ideas and then vote on them.
i would not just decide on my own.
have town hall meetings to see what the people of
the city needed most. then develop a list of possible
projects from that. get what the city needed most
the people of the city are who knows that.

Talk to inner city kids and see what they want to do after school. And HOOK THEM UP. Look at what Tiger Woods is doing...there are some decent people in this world.

A new water filter for the town.
I would be put down in history if I did. I live in a place where the hardest water is. it would be nice to have nicer water for the town.
