As you probably know, all cell membranes are made of fatty phospholipids. Because of this lipo-phobic substances, like ions, can not pass through the membrane. However a lot of endothelial cells, like the ones that make up blood vessels, are only loosely joined to one another. This helps white blood cells squeeze between the endothelial cells, and it also gives a mechanism for lipo-phobic/hydrophilic substances to pass to the other side of the endothelial cells: by squeezing between them.
The endothelial cells that make up the blood vessels that penetrate that brain are difference, they are very closely bound to one another with tight junctions, this means that hydrophilic substances can not pass between the endothelial cells, which excludes them from moving from the blood, into the brain.
On top of that, most endothelial cells that penetrate the brain are enriches with intergral membrane proteins along the lines of the multidrug transporter/p-glycoprotein. These molecular pumps are able to pump chemicals out of the brain. These transporters further exclude foreign chemicals (xenobiotics) from the brain.

Basically this just means that there are chemicals floating around in your blood stream that cannot pass out of the blood vessels into the brain tissues for a variety of reasons such as their molecular size or class, electrical charge, etc. On the other hand, there are some chemicals that move freely out of blood vessels into the brain. Overall the idea is quite similar to diffusion.
