A Urologist. They do it in the office as an outpatient procedure. The Urologist I work with says it is the most "no show" procedure they do. He says they usually call ahead to make sure the patient is planning on showing up and not changed his mind.

It's most commonly done in a Dr. office. It's actually a very simple procedure. The reversal requires a Urologist though.
