My friend got hit by a hardball square in the groin (when drunk) and after he recovered from the shot his testicles ended up swelling to the point were he could hardly move. Why do they do this? Eventually they returned to normal but should he have gone to the hospital???? Can you get hit so hard that they get damaged permanently?

Firstly, they swell, just like most other parts of your body, because of a buildup of fluid and a way of telling us that something is not right. Secondly, Yes they can get hit so hard that they are damaged permanently, such as when the testicle gets squashed between something and the pubic bone, because then blood can get out of the testicle and into the scrotum. Or blood vessels can be ruptured/burst and thus lead to more swelling and bruising/internal bleeding. And, yes you should definitely go to hospital if it doesn't get better in a few hours, or if you experience vomiting, fever or intense pain for more than an hour.
