Read the following case study.

You are a medical assistant working in a busy family practice office. One of the physicians says to you, "Please prepare a letter to go to Mr. Ford. He is the attorney for Ms.Smith. Just check her chart to get his address. Tell him that Mrs. Smith has not been coming to her scheduled appointments. Tell him that I can't help her if she doesn't want my help!"

Make up an address to use in your letter for the attorney and the medical facility.

Not sure what questions that you would want to ask before preparing the letter. But here's a quick example of what I would write.

Attorney Name
His Address
City, State, Zip

Dear Attorney Ford,

Your client Mrs. Smith has not been attending her scheduled visits. I am bringing this to your attention with hopes that you can speak with her and let her know that if she continues to miss her appointments then I will not be able to help her. Please do not hesitate to call me with any questions or concerns.

Respectfully Yours,

Doctor's Name
Medical Facility Name
Address
Phone Number

I wouldn't have any questions. The Dr. told you what he wanted it to say. Something like:

"Dear Mr. Ford,

This letter is to inform you that Ms. Smith has not been coming to her scheduled appointments. It is important that Ms. Smith continues to receive care to improve her condition.

Please call me at (333) 555-1212 so that we may discuss the situation. If I am unavailable, please leave a message with (insert your name here), my assistant.

Thank you very much for your continued cooperation in this matter and I look forward to hearing from you soon.

Sincerely,

Dr. John Smith"

Make sure you use proper business letter format. If you don't know what that is, use the letter wizard in MS Word. It will show you how.
