For car trips, get a Calming Cap. It covers the dog's eyes so that they can't see as well (they can still see, though). When movement is obstructed, the dog is not as reactive.

You certainly can use it on walks, too. For training, bring along something that your dog adores (bits of hot dog or his favorite toy, for instance). When a car is approaching - BEFORE HE REACTS - have him sit or get his focus on you. Reward him with the treat/toy. If he does react, toss the treats or toy at him. This should hopefully break his concentration so that you can get him to focus on you. Either way he should start to learn that cars = good stuff. And since you are the source of good stuff, he will look to you, not the car.

You might find it helpful to have a trainer work with you in the beginning to establish good skills.

Keep him on a leash. Problem solved.

As for riding in the car, possibly his deafness is causing balance problems and nausea in a the moving vehicle. Consult with your vet for possible solutions.

I'm afraid for your dogs safety you'll need to keep him leashed around cars since its very difficult to teach recall to a deaf dog...although its not impossible.
