Instead of using bread and making a sandwich, use a tortilla and make a wrap. I have found a brand of multi-grain tortillas (I can't remember the name right now) that has 6 net grams of carbs per tortilla after subtracting the grams of fiber. 2 slices of bread usually has around 30 grams. Drink water or sugar free drinks.

Try raw fruits and veggies or cooked. Salads are great. That's all I got. ={
