It is hot in your flask and milk doesn't like to be hot for too long.
When it is exposed to heat for a long period of time, the fat will seperate and you get curdled milk.

It's disgusting isn't it? And the smell when you open the flask.... eeww!
Take the milk in a separate container. A bit of a faff, but no curdling.

Because - I am almost certain you use freeze dried coffee. To avoid curdling make your coffee cool a litlle bfore you add the milk. That should sort your problem.

I think it could be the Milk as this happened to me before, but someone told me that you can buy packets of separate cup drinks ( like you get in vending machines ) which you can just add hot water too.

Well if its white coffee, that means it's got milk in it and milk goes bad pretty quickly.
