I was given a form (summons) to appear in court... do I have to take this form to my court hearing?

Nope. As long as you show up on the appointed date at the proper hour, you should be fine.

If you need validation, just call the court clerk's office.

No, it is not necessary. If you're not sure of the date, you can check beforehand with the court. If the duty solicitor is going to help you and needs details of the charge, then it is always possible to ask nicely for a copy of the charge sheet on the court file.

No its not neccessary to take it with you and how can you if you have lost it? As long as you know the time and date, if you are not sure ring the court and ask them.
