Of the US ones, there were none built on a battleship hull, but battlecruisers. The Lexington and Saratoga.

Other navies:

Battleships:
Bearn converted from a Normandie class battleship for France.
Kaga converted from a Tosa class battleship for Japan.
Shinano converted from a Yamato class battleship for Japan.
Eagle was built on the keel of the Almirante Cochrane which was being built for the Chilean Navy completed for the UK navy.

Battlecruisers:
Akagi converted from a Amagi class battlecruiser for Japan.
Furious converted from a battlecruiser to carrier for the UK.
Glorious and Corageous also converted from a battlecruiser to carrier for the UK.
Lexington and Saratoga converted from Lexington battlecruiser class to carriers for the US.

Of these only the Furious, Glorious and Courageous were completed in the battleship/battlecruiser before being turned into a carrier.

To my knowledge there are no US carriers built on the keel of a battleship. Our very first Carrier, the USS Langley, was built on the hull of a cruiser.

Most of the ones from WW2, they used the same keels for both.
