Ask your local council, they will give you a form and you then get the school to fill in their part, sent it back with proof of your low income.

Are you in the UK?
Talk to the school as they often have lots of as-new second hand stuff for just pennies.
Big supermarkets do uniform incredibly cheap too.

Every school has methods for assisting families who need help with this. Contact your school, no matter how embarrassing it is, and explain the situation. They will give you direct help or point you in the direction where you can get help.

You need to contact your local council or look on their website - they will send you a form to fill out - they should automatically send you the forms if you are entitled to it.
