As for the Sailor Moon story goes, in terms of the anime and manga, no there never was a Sailor Polaris or a Sailor Chibi Polaris. More then likely, as much of it has been done, it's a created character by a fan from a fanfic or whatnot, that often gets mistaken for a real character.
This also cn't be true because Usagi only had a brother in the storyline and no sister, and though the 5 TV animation series and the manga, up to the final Sailor Stars saga, that character never made an appearance.

Yes but i could be wrong im not sure what kind of answer your looking for, but ive heard the name inside Sailor moon. However out side of that i have never heard of that name. Sorry.
I think the name was mentioned in the 5th season of sailor moon. Hope that helps.
