Gerard said that it was the first song on the album that was really about redemption. It was written when Mikey was gone and when he was diagnosed with bipolar disorder.
I'm not exactly sure what was going through the band's heads when they wrote it, but the message I get is that even though you've messed up a lot in life, there's still something worth living for, and you need to keep living for it and be unashamed of yourself.

i got the itunes interview: "my chemical romance welcomes to the black parade". and on it Gerard said something like that songs means never giving up and that you can get through it. Also he said it was a great way to end the album because it leaves you with the hope that the patient might live.
so i think the songs is about that like hope and never giving up when things are rough.

the lyrics are pretty clear to me and thats what i think it means.

Hmm um well concidering that The BlackParade IS a concept album i'm guessing it's what 'thee patient' is saying to the listener or whaat is going on in his head.. just a guess on my part though...

good the complete black parade album has a tale line to it, its a thought album. when you consider that its on the finish of the album i idea it ment that the sufferer was once coming to phrases with the reality that he is going to die of melanoma and possibly he is residing his final few weeks or months being more potent figuring out any day might be his final.
