However, you might require a letter of recommendation from your employer in the future, so it's better to do it formally, by writing a letter.

2 weeks notice is a courtesy but not a requirement. Its good to give notice because you may want to go back after your school is over or your schedule changes. Or maybe you may need to use this job as a reference for a new job and you don't want a negative impact on a new employer for leaving without notice

Yes. in most jobs besides casual, you have to give at least two weeks notice before you quit and it has to be in writing in formal letter form. Keep a copy for your self. Your letter should say something like:
"To _________
I am writing to notify you of my resignation of (what ever position you are in) in two weeks time. I will be leaving on ___" or something similar.

You are best to give written notice, carefully worded and polite. That is do not burn any bridges you do not need to. Good manners would also indicate you when handing it over you should be prepared to discuss the issue with relevant people only.

Its customary to put it in writing. This way you have proof if there is a dispute as to when the date of separation is. Plus its just a common courtesy

Hourly jobs verbal is enough. A professional or white color salary job I would use written.
