They're called cigarette holders or extenders, and they're very hard to find. If you're looking for one for fun, I'd try a Halloween costume shop. If you're looking for a functional one, I found a couple you might be interested on Ebay

I think they are just called cigarette holders. You could probably find them in a tobacco shop
