I am making additions to an enrollment form for an after school program and want to ask for an email address but I want it to sound professional. Can anyone help me word this please?

To be professional is different from sounding professional. To be professional you will use plain English, so that you will be understood by everyone.
Bureaucrap: An email address is compulsory. Failure to provide this may be the cause of you not receiving vital information about the project.

Plain English: Please let me have your email address so I can keep you informed of what we're doing.

See the difference? Plain English is for everyone - bureaucrap is for politicians and public servants,

The best way to ask for someone's e-mail address is to do so directly. Ambiguous phrases, such as, "How may we contact you electronically?" will leave parents wondering what you mean.

Most people are willing to share their e-mail addresses with legitimate, familiar institutions so long as they're certain their address will neither be shared nor messaged too regularly.

I suggest a straightforward question such as, "What is your regular e-mail address?" and include a note that says, "[this school] will not share this e-mail with any outside company or institution, and will only contact you via this method regarding our after school program."

I don't get the problem.
Just underneath where you have blank lines for their address and phone have a blank labeled "your e-mail address" which also needs to be filled in.
