first right click the area where the address bar is located and uncheck lock tool bars....now that this is completed mouse over the dotted line to the left of the word address till you see it change to a double ended arrow left click and pull to the left and down just a bit....this should bring out your address bar where it is usable....once you get it back to the right place right click the toolbar and check the lock the toolbar line to prevent it from disappearing again.
Hope this helps.

Right click where it says address and uncheck the "lock toolbars" option. Now there should be a verticle bar you can "grab" with your mouse to the left of the word "Address" and you can drag the bar to the left or down to its own row.

Right-click that "Address" label you see there, uncheck "Lock the Toolbars" and then drag "Address" below the menu. Unless you wish to move it around some more, restore the "Lock the Toolbars" as good practice.
