My vote goes to Grave of the Fireflies ( Hotaru no haka ), a beautifully animated feature that deals with the slow death by starvation of two children orphaned by the firebombing of Kobe during WW2.
This film has become an icon for sheer unrelenting tragedy among long term anime fans, and is hard to beat for sheer tearjerking factor in ANY medium.

Well I'll agree that air ended pretty sadly, although I'll have to say the saddest I've seen would be Ayashi No Ceres.
Kind of hard to top a storyline where the main character basically has her family/friends one by one killed by a company that wants her due to her reincarnation. And by the end her closes family member, her twin brother, pretty much has to kill because he was reincarnation of an older spirit as well and to top it..the one whom she fell in love with, worked for the company doing all those horrible things, and by the end of the story he gave up his spiritual power so that she could live after her brother's reincanration just about kills her , so at the end of the story she has a baby with the one whom she loves but he says he won't live longer then a year [since he can't live long with out the mana he gave to her so that she and the baby could live]. Pretty sad to loose alot of people close to you and only because of being the reincarnation of a past being. T^T

I don't think anything can top Grave of the Fireflies as being the saddest anime.
Seita and Setsuko are struggling to survive on their own during WWII
in Japan after their house is bombed and thier mother killed in the process (although they don't find out until later)
Seita's father is in the Navy, sending home money to support them but that eventually stops because he is killed in battle. Now, the two children are on their own.

Fullmetal Alchemist is DEFINITELY the saddest anime to me. The Elric Brothers lost one parent, tried to bring her back, and almost lost another brother in the process. The daddy was never really there anyway. Other things make this anime sadder than it needs to be.

Just finished watching the new version of Kanon and it is right up there. How one person can go through this much tragedy and not totally lose it , but does hold on to a point.

I think the saddest anime was Rurouni Kenshin because first, Kenshin lost his parents. Kaoru too lost her parents. Yahiko too lost his parents. Sanosuke too lost his parents. And Misao and others characters lost their parents. But the saddest part is in the OVA when Kenshin is too sick and old and Kaoru too and she goes to see if he have arrived and and both see each others. Then he lean in her legs and she was talking to him but he was already dead, when she moves his hair, she noticed the his X-scar in his face disappeared.

i have to agree with b_bankhead, sugrnspyce4 and amarantine13.
Grave of the Fireflies.
this is the movie that my sibs and i have not watched in years specifically because it's so sad. it depressed you and makes you feel ashamed for not being able to help setsuko and seita. coincidentally, my sister and i finally watched it again tonight, as she's writing a paper on it for her japanese history class. and it's still just as heart-wrenching as before. it'll be put back on the shelf to collect dust for another 5 years or so.
