Do I have to inform my job about wanting to take additional month of leave I am entitled to 16 weeks of leave. The paper work i received from human resources is that enough. The paperwork shows the date printed to when i need to return.

If you want more time than the paperwork says you are allowed to take, then you need to request it, as early as possible. They don't have to give it to you, and if you don't return at the originally scheduled time, they can terminate your employment and you would probably NOT be eligible for unemployment comp.

If the date on the paperwork says the date you actually plan to come back, but there were verbal conversations about coming back earlier, then call HR and talk to them about it - coming back on the date on the paperwork is probably OK, but you do need to let them know.

the normal delivery as per rule by SSS maternity leave is up to 60 days while for caesarian delivery the maternity leave is up to 75 days only. Most of the companies follow this ruling but there are some to the extent they will allow more than the minimum required.
