Episode 4 of season 2
"Children Shouldn't Play with Dead Things"
Near the end of the episode, when the dead woman was chasing Sam through the graveyard she jumped on him pushing him to the ground.
Unfortunately he really hurt himself doing this stunt.
You can see a flash of him writhing in pain on the ground as Dean races past him to impale the woman in her coffin.
