Depends on what kind of plant it is. It could need fertilizer, a change in the amount of light it receives, you may be overwatering it, or it could have become root bound. Are there any roots coming out of the bottom? If there are then it is definitely root bound and it need repotting. How long have you had it? If you've had it more than a year you probably need to repot it.

Which means you would need new soil (preferably with time release fertilizer mixed in) and a new pot (larger than what it is in now). Make sure you wash the post with antibacterial soap and bleach and let it dry. Also make sure you are using new soil and not recycled soil. You would need to take it out of the pot and break up the root ball. Feel free to hack it apart with a trowl and knock the dirt loose from the roots so it looks like a hunk of grass you might pull up from your yard. Put clean rocks in the bottom of your pot and fill it about 3/4 with soil and then put one of your sections of your plant in it and fill in the dirt so you have about an inch of free space to the rim. Pack down the soil and thoroughly water the plant. It will have issues for a couple of weeks and then it should have re-established itself and will be much happier.

It may also need a different kind of fertilizer depending on the species of plant. Fertilizers come in combinations of Nitrogen, Phosphorus, and Potassium (Potash). Talk to your garden person at Lowe's or Home Depot and they should be able to tell you which combination you need.

What kind of plant is it?

Too much water especially if the pot can't drain after watering will kill it.

Tap water will kill or hurt some potted plants but very few.

It might be sick, does it have mold, mildew, bugs or spots on it?

Some plants like alot of sun some only like shady areas.

In order to help your plants live you need to know about what that specific plant likes a dog and a cat are both house pets but if you tried to take care of them the exact same way neither would be happy.

Well you didn't give me much to go on about what the plant might look like. Some plants like to indirect light which means not the window sill where it might be too hot. Some plants need different watering schedules some take little water like a cactus plant .To be able to give you an answer that might help, please give more details.
