Keep it simple. Soft sugar free candies. Put in a puzzle book to keep the mind active. Finger nail polish to make her laugh. A nice lap blanket. A small framed photo of you or the family. A coffee or tea cup with a small plant that she can replant and see grow. House slippers.

Fruit might be even better than sugar free candies. Of course, you know your own grandma better than we do. More handmade items would probably be your best bet, unless there are purchased things she seriously needs, because the handmade things would mean more to her. You could try making the eyeglass leash (or whatever those necklace-looking things that reading glasses dangle from are really called) if she uses reading glasses. A family photo is an excellent idea. Does she use a computer? A nice CD of family photos would be nice, if she does (some folks that age do, others don't.) If she doesn't, could you put a few photos in a small album for her?

The chocolates suggested by other answerers are great ideas. You could also get some plastic easter eggs and print some coupons to fold up and enclose in the eggs.
Suggestions for the coupons:
A coupon for a haircut and style.
A coupon for a manicure and pedicure.
A coupon for lunch with you at her favorite restaurant.
A coupon for a home-cooked meal is ALWAYS welcome!!
A coupon for a book by her favorite author.
If she likes to send greeting cards, put a small box of cards inthe basket.
A coupon for you to help her update her family address book.
A sheet of postage stamps folded into one of the easter eggs.
If she still lives at home, a coupon for yardwork and mowing the grass.
-Notes from you and other family members telling her how much she means to all of you.

It is small things like this that mean so much to elderly ladies (and men). I know because I am one

Get her sugar free chocolates,favorite lotion,slippers,gift card from favorite store,small photo album with an IOU in it(you owe her photos of Easter 2007)disposable camera,Easter mug,coffee or teas,and your undivided attention for 30 mins of that day..Happy Easter...
