If you are US-based, asking a prospective employee for a medical history or access to records is illegal except under very specific circumstances. The company may ask you to provide a fitness-for-duty certificate and/or take a test (such as a drug screen) AFTER you have accepted a bona-fide offer of employment only.

If you are performing a job (such as an airline pilot) that requires the employee to have good vision, lift heavy weights, etc., you may be tested for your ability to do those things - but that's all. I can't think of a circumstance where it would be ok for an employer to view your medical history.

No, you don't have to give permission. But they don't have to give you the job either, and would very likely not if you refuse access to medical info. You should talk to them - they might not be looking for your ENTIRE medical history, just certain areas. Requiring you to pass a physical or to fill out a medical questionairre before you start a job isn't unusual - requesting access to all of your medical records isn't as common.
