He sounds quite sick.
A hospital tank with 1 teaspoon of pure salt and 1/4 teaspoon of epsom salts to help with osmotic function combined with a methylene blue bath using the tank water in a 16 oz. container for 30 minutes can be effective for many bacterial and parasitical problems.
Kanamycin would be a good absorbing antibiotic to use in the tank.

If he is tilting to the side (is he lying on the bottom?) it could be a simple case of constipation but could be swim bladder. You could try isolating him and feeding him a cooked green pea (skin removed). That will cure constipation. If it is swim bladder, there isn't much you can do for him.

go to the pet store and get him medicated fish food and then cover his tank with a sheet for a day or two to avoid as much stress as possible. Have had 4 gold fish i won at the fair that lived for 5 years. I am kinda a fish whisperer!!

mine was acting strange whne I first got him. Feed him freezed dried blood worms and look into Swim Bladder remeidies and he'll prolly be fine

our fish was on its side
we went to pet shop and brought
all purpose medicine for $6
the fish is great now
it does fin rot, white spot and helps inside the fish
