I know it needs to be a notorized letter. But do I need my drivers liscence information, social security number, name, and address? Or what exact combination of the above do I need? Thanks in advance.

I was told by the sporting goods store, that it could be for any number of reasons... but I needed to ask the ffl local office for details. Could be for unpaid parking tickets. Or if a person had been dishonorably discharced from the internet. Thanks for your answers.

And I must be terribly tired, I need to read over what I write before I submit it. The sporting goods store never gave me the details on what information that the office needed for the notorized letter. I haven't quite found out why they've denied me yet and I was just curious what information they needed in the letter. Thanks again for your help.

Click on this link to the official NICS appeal form and follow the instructions exactly:

One thing you will need to get from the firearms dealer who processed your original application is the "NTN", which stands for NICS Transaction Number. (NICS stands for National Instant Check System)
