How do they go about considering a county or a city? Is any county always country or are their counties that are just like big cities? If Its a country area is it always considered a county or town?

A county is just a smaller division of a province's or state's area. A city is a concentration of population. You could have a county which is a city, but i don't think it's very usual. You could also have a city straddling the border between two counties.
