The built-in flash has a very limited distance, and if your subject is more than about 12 feet away, the flash isn't strong enough. The external flash will extend this distance; by how much depends on the model you buy: check the specs on Canon's website. Even if you're just walking around the tables at the reception you might want a stronger flash. At the very least, with a better flash unit you'll have more options.

What you rally need is a bracket that places the flash up and away from the lense for better shots. THere is a swivel fram that let's your camera rotate while the flash stays in the correct position, sweet.

A braket like this can make any flash better, but you might wanth to invest in a well tested flash for your unit, with lots of range.

If you just plan on taking "snap shots" of a friends wedding then no. If you are planning on doing a serious job of it, then a good pro flash is a must. I use a Vivitar 283 Pro on my Pentax DSLR as well as on my Miranda 35 for most flash jobs, not just weddings.
