I would also check her anal glands. They are glands inside of her bottom that fill with fluid. What normally happens is that the dog will empty the anal glands with each bowel movement. But sometimes, more with small breed dogs and older dogs, the anal glands don't get emptied. That makes things very uncomfortable for your dog. Usually when you see a dog licking, chewing a lot at their backsides, or dragging their butt along the floor, it is an anal gland issue. A groomer or a vet can empty those for you. I do not suggest doing it yourself as it is very nasty stuff!

Your Pomeranian is 12 years old. Its time to say good bye. This is not a smart answer but reality. If you have treated her for fleas and she hasn't gotten any relief then we have to go with a nasty skin disease (maybe mange) or some internal problem that is getting to the non-tolerable point. Its time to evaluate for quality of life. If she has been with you since she was a puppy then I would guess she's had a wonderful life. You could set yourself up for trip after trip to the vet and never be sure that your beloved pet is pain free from now on.
My min poodle started out like that at age 12 and it was all down hill from there. I had to fly home from Calif to have her euthanized. My mom wouldn't do it. She saw Cindy every day and she didn't notice all the problems showing up, but I hadn't seen her for years and it was obvious to me that she wasn't well and at age 12 little hope of regaining her previous good health.
I gave my mom a beautiful white westie puppy and they were buddies in no time. I purposefully chose a westie because it looked nothing like the black poodle. I knew that she loved them from years of watching the dog shows on tv. Bonnie had a heart attack at age 11 and even though we medicated and pampered, she died suddenly just before her 12th birthday.
I gave her a beagle on loan after that because she was thinking of moving into a senior complex with no dogs allowed. Betsy Mae kept my mom company for a while and when my mom moved into the apt, Betsy Mae Beagle went to live with a family that had three little kids and plenty of love for a tail wagging beagle.
You may want another rescue pet. Maybe this is a sign that another pet needs you. I don't know. I only know that for dogs, life after 12 gets pretty painful. ((hugs)) and Keep the Faith.

Keep checking for fleas.
Also at 12 years old she may not be able to get herself clean on her "bottom" after having a bowel movement. She might be trying to "itch" it. Check out her "bottom" and see if it's dirty. You can use a baby wipe to clean up any nasty bits. Also check for worms or any abnormalities in the poop.
If the flea bath doesn't solve the problem take you pom. to the vet and get it checked out.

If you have been away for a while nad came back to see that you dog is acting funny its probably due to stress because of a change in environment or anxiety problems. Also if your dog is on the older end of the age scale it could be due to a change in natual hormones(just like we go through) that are making her skin very dry and sensitive. I have always cured that by adding a little vegitable oil (not much, just enough to VERY LIGHTLY coat ) on my pets dry food. It may take a few days to notice a difference but it has always worked for me. You should also make sure that its not a mite(little "bug" that burrows under the skin).

This could actually be a few signs of numerous problems... Ranging from fleas, to Rabies. Not to scare you or anything, but I would suggest watching for any other signs of illness, such as a dry warm nose, watery / goopy eyes, or anything out of the ordinary. Pomeranians are not really known for their long lives, and seeing as she is already up into her years, her age may be the biggest factor.

Taking to the vet is the best...fleas can be controlled by the back of the neck drops....but it is also important that she not have any other reason for the trying to get to something bothering her. Sorry for your situation...she has no other way to say "this is really bothering me".

I would be concerned that something is going on inside. Could she be constipated? We had a cat who we had to take to the vet to be "cleaned out" a few times. Maybe they used a catheter during her surgery and s/he is still a bit uncomfortable. Call your vet for advice!

apply frontline for fleas as directed.... if you found 2 fleas,,, THERE are more... flea baths don't work,, as the fleas spend about 80% of their time off the dog....
