My first tattoo was a koi in that precise area. Depending on the size you plan on, it can get over to fairly sensitive areas - for a lot of people the mid-center back is a more painful place. The upper shoulder, at least until you get to bony parts, isn't that bad. All things told it was a non-trivial test of my tolerance, but I keep coming back for my current expansion so I guess it wasn't that harsh.
Keep in mind that endorphins kick in within 5-15 minutes anyhow; how it first feels will not be how it feels once you're into the process.
Mine was a fairly large first tat (I'm told), and I was in the shop for a little over 3 hours. Some of that was the initial temp image to my skin and prep work (cleaning the skin, etc). It's on my 360 if you want to compare sizes to what you're planning.

It depends on the tolerance you have for pain. I have one on my right shoulder blade about the size of a large fist. It hurt...but all tattoos hurt. Anywhere where there is bone the tattoo will hurt. It's less pain if it were just fatty tissue...say your belly or something. I suggest listening to your iPod or something. Keep your mind off of it. First timers get scared at the sound of the needle so if you can't hear it you won't make a big deal out of it. Also the vibration of the needle is a lil much but keep still or the artist will dig in more which of course hurts. Make sure to tell the artist it's your first time and you may need a minute between things. If (s)he too busy re-schedule to a slow day. Time is necessary for a good drawing and experience.
I do have some advice though:
Don't get it until you know what you want and what you're getting yourself into. I love tattoos so I won't try to talk you out of it but what I see more and more these days are stupid people getting a tattoo as a fashion statement and regret it like a month or year after. Which just makes the tattoo community go down a notch.

Well I got my first tattoo on my "right" shoulder blade about a week ago and it hurt way less than i thought it would. It's more like a pleasure pain, but it hurts the worst when the artist has to stop and begin a new line. It all depends on how big your tattoo is going to be...but you will be fine :-)

That's a pretty tough area right there, but the closer the gun gets to your shoulder blade (the bone), the more sensitive it will become. It hurts a lot less than say, the foot, or the base of your spine. But you also have your own pain tolerance, and only you will know what it is when you get started.
It also depends a lot on how much work you're getting done there. The one I've got took less than 3 minutes to complete; relatively small and only one color. It was quick and pretty painless; I couldn't believe my artist was done when he said he was. The more area the gun covers, the bigger the painful area will be.

I just got one 2 weeks ago on my right shoulder blade, im sure there is no pain difference for the two...but anyways, I didn't have an issue with too much pain. I got a medium sized butterfly done. The outline hurt, but it also kinda tickled. I have a very low pain tolerence, but I went through it very easily. Its different for everyone, so don't take our word for it, you just have to do it. The guy doing my tat did a small line for me just to show me what it would be like and then stopped and asked how i felt. So you could tell your artist how you feel and they may help you through. Also it helped me to bring someone with me. The three of us talked and it kept my mind off of it. Good luck and Hope you like your tatt! :)

I got a flaming butterfly on my shoulder blade, mine is more on the muscle part closer to my neck. This was my second tat. My other one is on my ankle. I think the ankle hurt more then the one on my shoulder. It probably took about an hour and a half and once they started to color it in the area went dead (not much feeling, but not numb). The only time it bothered me was when I was driving home due to the seat belt and that part touching the seat. Other than that it wasn't so bad.

I've never gotten one, but I know they use a needle to get the ink in there. Since it's on your shoulder, it might hurt a little more. It also depends on how big the tattoo is and the colors, the deeper the colors, the more pain... Hope this helps you

All tattoos hurt. That's how you "earn" it. I have several, though, and found the ones on the upper back the least painful. A general rule of thumb is to stay away from bony areas.

The closer to the bone the tattoo is, the more it will hurt. If you want to avoid pain, I suggest your calf since it's meatier than your shoulder blade.

much more than the right shoulder blade HA *snare drum and symbol crash*
horrible i know sorry. couldn't help it. actually, your should will do well with pain. if you're real thin and the needle touches your bone it will be a shakey experience, but regardless it would hurt A LOT WORSE if any where else.
