Yes, you should be able to. However, you need to check your front sight. If the front sight is either dove-tailed or staked into the slide, replacing the front sight should be rather easy. However, if your Smith & Wesson front sight is an integral part of the slide, this could involve machining the slide.
If the rear sight is coming off, this should be covered under warranty with Smith & Wesson.
You should be able to purchase adjustable sights with replacement front sights from Smith & Wesson. You can also get replacement sights from Millett Sights. You can find their sights, along with other manufacturers, at Brownell's (http://www.brownells.com/aspx/NS/Home/default.aspx ).
If the front sight needs to be staked and you are not equipped for this, you will need to employ the services of a gunsmith. If the front sight is dove-tailed you should be able to do the sight swap yourself. If the front sight is integral to the slide, and you can not find a rear sight that uses the fixed front sight (for height) then you will either have to find a VERY GOOD gunsmith to do the machining or send the pistol back to Smith & Wesson or their custom shop for installation.

Just a suggestion in case you haven't already tried this. Before you spend a bunch of money on new sights, labor, etc , try different ammo. It can make a big difference.
