Because the surgery itself puts bacteria into your system that increases your health risks immensely.

If you have an autoimmune disease, you're going to find that any surgery is very much more dangerous for you than it is for someone with a healthy immune system.

Your eye is your only exposed organ, that makes it more suseptible to infection. If you have autoimmune disease and therefore a weakened immune system, your body couldn't fight off the infection that you might get in your eye. Then the infection could spread (maybe to the brain too, I'm not sure though) and you would lose your eyesight (and most likely your eyeball).
