If you answered 6 months then perhaps you could cook him a terrific home-made meal. But if you can't cook, or he's a chef, try something else.
All joking aside, the key to doing something special for him, is discovering the answer to: what does he like? and not what would you think is special.
Let me give you an example: My husband had a book written by his favorite hockey player. I tracked the hockey player down, sent him a letter asking him to sign it, and the guy signed it and returned it. Another time I hunted down a rookie card on this hockey player and gave it to my husband. Another time I had a custom surf board made for my husband according to his specifications.
Give some thought to what would be meaningful to your guy, an autograph from his favorite athlete, a hard to find vinyl record of his favorite musician, a night on the town doing things he loves to do, but you don't particularly like. Show him that you listen to him, and that you want to make his dreams come true, no matter how big or small.

i see your new to this website.....ok....i'll give you my true words...set up a breakfast at a nice restaurant,talk with the manager their to sing a happy birthday breakfast song for him...also,make a large pancake as the birthday cake with strawberry,and blue berries as the topping and add lots of cool whip cover all of the pancake..lit some candles and surprise him....try something new and out of the ordinary....if,you go out at night he might think something is up...morning is different and he will just think its just another breakfast eating day..........i personally that is super sweet for your guy....later on in the day,he will have his super delicious dessert from you>>>>>hehehe.......hmmmm wow.....

i suggest you write him a nice card. buy a blank card from Hallmark and pour your thoughts into it.
then buy him a book on something inspiring, like success or happiness.
i know it's sappy, but it works. my gf bought that for me. i liked it.
