You can go to a teacher supply store and get a homeschoolers log book/grade book. Some of these have templates with transcripts in them. If not then order your transcripts from your high school or college. Hope this helps. If not try to contact one of your local home school groups and they should be able to refer you to where you can get the information necessary if you are trying to fulfill a requirement.

Your transcript is a record of your grades not your academic record that consists of other details such as detentions and other things.

Classes and grades. if you are homeschooling, you have some leeway in what you put. Contact the universities you want to attend and ask them what they'd like to see.
