Chocolate comes from the cacao seed which is mainly grown in South America,which is then dried ,fremented and then sold on the Coffee,Sugar and Cocoa Exchange. it is then processed to make cocoa powder and chocolate.

Not in plant form, but once you break down the cocoa, it will then turn into dry cocoa substance, then you can actually make hot chocolate or any kind of chocolate.

chocolate must contain cocoa butter, even if it's dark chocolate with no sugar, while cocoa powder is cocoa beans from which was extracted the cocoa butter
