Frozen ground beef can go from the freezer to the pan, but it's not recommended in most instances. Freezing turns the natural juice in the beef to ice crystals. If you cook the beef while frozen, chances are you'll be draining off the juicy flavour along with the fat. When possible it's best to defrost it first.

It is safe, but it will seriously affect the palatability of your meal.

Fry off your onions, then add the mince, and reduce the heat to medium-low. Keep stirring the mince and poke at it with a wooden spoon until the mince is broken up fully. Add your other ingredients, BRING TO A SUSTAINED BOIL, then cook as normal.

Well, if I understood you well enough I am sure you thawed it fiorstr before frying. But I would never buy Frozen ground meat of any sort. You don't know what the heck is in it. Firstly I am sure it is a bunch of leftover unsold meats and secondly it would have a lot of grounded fat in it. I prefer asking the butcher to ground some fresh meat for me.

Are you talking GROUND beef? If so, yes its perfectly safe. I do it all the time. In fact its probably safer to do it this way than to allow it to defrost (thaw) first because bacteria won't have had a chance to multiply on the thawing meat before cooking.
